HELP! Radiator drain plug has come out TWICE!

OK, so I get out of work yesterday, and I see a big puddle of coolant, along with a drain plug lying on the ground.

So, I replace the thing and fill the radiator. Then, the car starts overheating AGAIN! And the plug is gone!

What is causing this?!

1) plastic drain plug threads are stripped, 2) plastic drain plug is broken or deformed, 3) radiator drain plug threads are stripped. I suspect Items 1 or 2. Just get yourself another drain plug and replace the original. In fact, get 2 drain plugs; keep one in the car for emergencies.
